Diabetes can affect many areas of your health. That’s why your healthcare provider may have put together a diabetes care team for you.
The role of these professionals is to make sure that you receive the most current and best diabetes care. However, the most important person on your diabetes-care team is you. Your role is to make sure that the care plan meets your needs and that you can follow it. Together, you and your team will work to adjust your diabetes care plan to your personal physical, emotional, and living situation.
